Birthday Party Request Form

We love doing Birthday party for little ninjas! It is an hour and a half long. We will:

• Do a short Taekwondo lesson
• Play games
• Break a pinata by kicking it (if desired)
• Cut the cake with a sword
• And open presents

It is fun. It is memorable. And all the kids have a great time!

We provide tables, chairs, disposable tablecloths, and any of the following you indicate that you would like to have provided:

• Plates
• Napkins
• Forks
• Spoons
• Cups
• Pinata (with Candy)
• Sword for cutting the cake

Please indicate below things you would like provided.

Additional items we do not provide that you may want to bring: 

• Cake
• Ice Cream (and a scoop)
• Drinks
• Party Favors
• Other food items
• Candles (and matches/lighter)
• Any other serving utensils
