- Tuition is due on Friday before the upcoming week. A $10.00 late fee will be assessed if paid after Monday morning at 9:00AM.A $30.00 returned check fee will be imposed for any returned checks. This program is not licensed by the State of Georgia, nor is it required to be licensed by the State of Georgia. This program is covered by liability insurance.
- I understand that Lewis Global Karate, LLC is a martial arts school and not a daycare in as such; their stock-in-trade is not supervision and care. The intent of Lewis Global Karate, LLC is to teach martial arts physical and philosophical character building skills. Additionally, if my child(ren) stays at Lewis Global Karate, LLC it is because of my direction and not Lewis Global Karate, LLC

WAIVER & RELEASE: Buyer and Student(s) agree that Student(s) is engaging in physical exercise, the use of equipment, and the use of Lewis Global Karate, LLC training and instruction facility, which can be dangerous to the Student(s) and could cause injury to Student(s). Student(s) is voluntarily participating in these activities and Buyer and Student(s) assume all risks of injury to Student. Buyer and Student hereby waive and release any claim or right to sue Lewis Global Karate, LLC, employees or agents for injury to Student(s). Buyer and/or Student have carefully read this waiver and release and fully understand, it is a release of all liabilities and damages to Lewis Global Karate LLC because of any injury that may occur. Lewis Global Karate, LLC will make no evaluation or recommendation whether Student(s) or guests are physically fit for any exercise activities. It is always advisable to consult your physician before undertaking a physical exercise program, particularly martial arts activities.

LOSS/DAMAGE/THEFT OF STUDENT’S PROPERTY: Lewis Global Karate, LLC does not assume any responsibility for the loss, damage or theft of any property belonging to the Student and Student agrees that Lewis Global Karate, LLC and its personnel are not responsible for or liable for any such property even if its loss, damage, or theft occurs on or about Lewis Global Karate, LLC facility.
- Buyer understands this is a legal and binding contract that must be satisfied in full, regardless if the student attends classes or student does not attend classes.
- We require a 30 Day notice to cancel. Written documentation is required.
- The supervision and care of the children are incidental to their participation in the activity or training in Lewis Global Karate, LLC. The program provider is not assuming responsibility for the provision of daily child care outside the scheduled program.
