LIABILITY/WAIVER FORM
I, the undersigned, understand that participating in martial arts classes and related activities at Tempest Taekwondo involves certain risks and hazards, including the risk of personal injury. In consideration of being allowed to participate in these activities, I agree to release and discharge Tempest Taekwondo, its instructors, staff and affiliates from any and all liability, claims, demands, actions, or causes of action that may arise out of, or in connection with, my (or my child's) participation in martial arts classes.
I understand and acknowledge that the risks associated with martial arts training may include, but are not limited to, the following:
Risk of physical injury, including but not limited to, sprains, strains, fractures, dislocations, and other musculoskeletal injuries.
Risk of contact with other participants, including accidental strikes, impacts, or collisions.
Risk of injury resulting from the use of equipment and facilities.
Risk of injury resulting from the actions or negligence of other participants or instructors.
Risk of injury resulting from the physical demands of the activities, including strenuous exercise and conditioning.
I understand that Tempest Taekwondo takes reasonable precautions to ensure the safety of all participants, but it is not possible to eliminate all risks. I acknowledge that I (or my child) am participating voluntarily and that I am responsible for my own safety.
I understand that Tempest Taekwondo is not responsible for any personal property that may be lost, stolen, or damaged while on the premises.
I acknowledge that this waiver and release of liability is binding not only on myself but also on my heirs, executors, administrators, and representatives. I further agree to indemnify and hold harmless Tempest Taekwondo, its instructors, staff, and affiliates from any liability, claims, or actions arising from my (or my child's) participation in martial arts classes
I have read and fully understand the contents of this liability waiver, and I sign it of my own free will.
